How can I approach a coworker about their body odor issue?

mmmmmm tough one , calls for tact. How about saying to your boss "Hey smelly ahhh , I mean boss" or "Hey boss why dont we take a nice hot bath together sometime ?"or "I just got a new bathtub ,would you like to try it out?"or "Hey boss you can put hot water in the bathtub you know"or "Boss is your bathtub out of order ??" Yes tactful comments along those lines should work quite well !!

Okay the simple way is to say " Are you aware of your BO problem?" alternatively you can carry around a can of deo and every time she is not looking give a squirt under her armpit. If she catches you, just say " Oops sorry I was aiming for my own armpit!

When she is away from her office, spray the underarms of her jacket with something that will deteriorate the material. When the armpits of everything she owns are rotted away, it might come to her without you having to say anything.

buuy your boss, a gift basket of different bath products, bath salts, lotions, bubble bathes, perfumes, and a special fragrance deoderant. If you don't want to give it to her just put it where she can find it with her name on a card.

Buy him/ her a deoderant OR If he is male ask another elder male to talk to him in a nice way. If she is a female then ask an older lady to talk to her. Use air freshners & talk in general about how you would cope with Hygene problems yourself. Jesus with you.
