I would first start with taking her to the vet to rule out a urinary tract infection. If that is clear then you may have to crate train.

Well it depends. She could be trying to mark her spots, especially if there is another dog in the house. Try a crate or kennel, keep her in there at night and during the day when you're gone. Make sure as soon as you take her out you bring her outside to go potty. Let her go for a few minutes, some dogs have to potty in multiple spots before they are finished. She'll learn after a few days to a week with that.

how old is the dog? Is she fixed? Are there male dogs in the area if she is not? Whats her breed? Is she left alone a lot? Is she bored? Or is she just trying to upset you? There are many reasons why a dog urinates in the house but this might be that the muscles around her bladder are weakening. That's why I asked her age also has she had large litters. If she is a young dog and there are no medical reasons why she would be doing this then you quick answer is, training, repetition, punishment(for most dogs this is a stern voice or quick movement with a growl NOT BEATING), and you being the one in charge at all times. Good luck I hope your faithful friend is okay.

She probably needs to see a vet to determine if she has incontinence issues.
